This form is to be used if you are applying for your 1st District License, New District License, Ordination Candidates at the Virginia District Church of the Nazarene and/or for Elders & District Licensed transferring in from other districts. We are not able to send background checks to any third party including your local church.

All 1st District License, New District License & Ordination Candidates are required to complete a new background check and Ministry Safe training. If Ministry Safe training was completed within the last 360 days at the district level, you are not required to complete training at this time.
